Eligible individuals or their substitute decision-makers receive funding to hire and manage care providers directly, including PSWs, nurses, occupational therapists, and physiotherapists. The program offers flexibility in scheduling and provider choice, with monthly financial reporting to Ontario Health atHome. PrimePath Community Care is an approved FMHC provider.
